Please complete this form on the first day of your child's absence from school before 9.30am. Include the following details within your message: - Your child's name - Specific symptoms of illness - Whether a doctor or pharmacist has been consulted and their advice. This information will be shared with the class teacher and will act as a written absence note.

Planned Absence

If your child has to attend a medical appointment or another planned absence, please complete this form as soon as you are aware it or by the day before the absence at the least. Include in your message: - Name of Child - Reason for Absence including time and date of absence. This information will be shared with the class teacher and act as a written absence note.
